In order for you to have access to the image database for this project, it will
be necessary for a permanent faculty member in your department to serve as the
investigator for your site. This person would assume responsibility for the
proper use and safekeeping of the images. There are two items which must be
submitted: (1) a signed copy of the participation form (available in PostScript or PDF
format), and (2) some biographical information. This latter item can be a
shortened (two-page) version including the following information:
Name, position title, education, positions held, and publications in the last
three years. This is the standard biographical information required by our
Institutional Review Board to allow investigators access to the patient
datasets used in the project.
